Scotchmo will be a special dialogue interaction, and upon your first meeting you can use First Aid 3 to point out that he's showing signs of late stage liver disease. He will appear at the Peterson Air Force Base afterwards. Scotchmo will be first encountered at The Bizarre, and will become available for hire once you complete the refugee quests there.
